The AHF Healthcare Centers provides comprehensive care access through our Medical Care Coordination (MCC) program, which is funded by Ryan White Part A and overseen by the Los Angeles County Department of Public Health (DPH), monitored by the Division of HIV and STD Programs (DHSP).

Alongside our specialized MCC services, the El Monte Health Care Center offers fully integrated Outpatient Ambulatory care, clinical laboratory services, and expert benefits counseling to ensure patients can navigate their medical insurance seamlessly.

To maximize our impact, AHF pairs these direct Ryan White Part A grants with flexible 340B program savings. This unique combination allows AHF to actively invest in our facility infrastructure, expand community outreach, and provide free safety-net wellness testing across the wider Los Angeles County region.

Proof of HIV Status: Lab report showing a positive HIV viral load, Western Blot, or a signed medical diagnosis form from a licensed Physician

Proof of Residency: Utility bill, lease agreement, or official mail showing you live within the state or county service area (unhoused individuals can submit a letter from a shelter or caseworker
Proof of Income: Recent Pay stubs, W-2 forms, tax returns, or a notarized letter of support showing your household income falls below the local limit.

Income limits in California: Your income cannot exceed the Federal Poverty Level (FPL) Percentage of 500% $79,800.
